Remote Psychiatry in Utah
Thoughtful, affirming care that meets you where you are.
A boutique psychiatric practice offering personalized psychiatric evaluation and medication management designed around your life. Longer appointments. Fewer patients. More time for you.
Psychiatric care should feel like a conversation, not a countdown. If you've felt rushed through appointments, overlooked by the mental health system, or unsure whether your provider truly understands your experience, you already know how exhausting that search can be.
Sage Meadows exists to deliver a higher standard of care. I work with people who have felt marginalized because of trauma, identity, or neurodivergence to build stability and self-trust through slower, collaborative psychiatric care that centers the whole person—not just symptoms or diagnoses.
“Care is intentionally structured to support depth,
continuity, and real growth over time.”
Meet Your Provider:
Hi, I’m Erica Chustz, PA-C, a psychiatric physician assistant with over seven years of experience treating adults across a wide range of mental health conditions.
Before founding Sage Meadows, I worked in diverse high-acuity psychiatric settings, including emergency psychiatry and adult community outpatient care. Those experiences shaped my clinical confidence and my commitment to thoughtful, individualized, unhurried care.
I founded Sage Meadows Mental Health to bring that level of care into a setting where patients feel seen, supported, and given the time and attention their care deserves. I help patients feels more stable, understood and grounded in who they are.
What Makes This Practice Different:
-
90-minute initial evaluations and 30-minute follow-ups, giving time to understand your story, answer questions, and make thoughtful collaborative treatment decisions
-
A limited caseload with responsive messaging, urgent visit availability, and a consistent provider who stays familiar with your life and treatment
-
Fully remote care via secure video visits from anywhere in Utah, on a schedule that works for you
-
Self-pay, out-of-network model offering more autonomy, fewer administrative barriers and greater privacy in your care
-
Psychiatric care grounded in trauma-informed, identity-aware, and neurodivergence-affirming practice—especially for people who have felt misunderstood or overlooked in traditional mental health settings
-
Shared decision-making in medication management, where treatment options are explained clearly and your preferences guide the final plan
-
Extensive experience across high-acuity and complex psychiatric presentations, including trauma, mood disorders, and co-occurring conditions
